Hamilton Health and Social Care Forum (HHSCF) released their first co-designed Positive Pathways to Mental Health and Wellbeing document for the Hamilton Locality last year.
The document was launched during Mental Health Awareness week and received a very positive reception from both partners and communities. Hard copies of the document were released in November 2021 with a downloadable PDF (attached) continuing to be available online at https://www.communitylinkssl.co.uk/positivepathways.
HHSCF are keen to collect your opinions on the document and identify whether it has allowed individuals and partners to access beneficial services. They would be grateful if you could complete the following short survey (link below) and give them your views on the pathway.
